Every write an owner makes produces an entry. So does every refusal — which is the part most records miss, and the part that answers "I tried that on Tuesday and nothing happened". The entries are written as things a person did, not as rows that changed.

That sentence is worked out from the entries above, not asserted. If anybody outside the firm ever acts inside this instance, that entry appears in this same list with their name on it and the reason beside it.
Two of those entries are refusals — the eleventh invite and the colour. Nothing changed either time, and both are written down anyway. A record that only holds successes cannot answer the question people actually ask it.
Undo appears only where an undo is real. On the entries where nothing can be put back, it is absent rather than present-and-broken.
